Jack M. Curtis M.S. Chief Executive Officer
Jack Curtis is founder and CEO of Corporate Health Partners. With more than thirty years of business experience, Jack is known for his high-energy leadership, analytical ability, team-building and organizational skills. He is instrumental in engineering the measurability of all CHP program core components and lead in all analytics and ROI reporting for CHP clients. Prior to founding Corporate Health Partners in 2002, Jack was President and Chief Operating Officer of a multi-facility medical firm that performs executive physical exams. Jack previously also served as general manager of a $50 million Berkshire Hathaway business unit. He earned his B.S. in Biomedical Engineering and his M.S. in Engineering Management from the University of Tennessee.
Jeremy Curtis VP / Partner
As VP/Partner of Corporate Health Partners, Jeremy Curtis is actively involved in client development, broker relations, marketing and business planning. Prior to his tenure with CHP, Jeremy leveraged his sales ability to become General Manager for a regional headquarters of a B-to-B distribution company. Under his leadership, his region increased revenues ten-fold in less than three years and became the highest selling and most profitable arm of the corporation’s $25 million business. Prior to that, Jeremy was a mentor and sales team leader at Dell Computer, where he consistently sold in the top twenty-five percent of all consumer sales reps. Jeremy also has experience in the fitness equipment industry, selling direct to consumers for two years while training them on the benefits of cardiovascular exercise and weight training. Jeremy has a B.S. in Economics from University of Tennessee in Knoxville.
Pattie Martin, MS Senior Account Manager / Privacy Officer
Pattie Martin started her career developing a faculty/staff and student wellness program for Eastern Illinois University in conjunction with a new Campus Recreation facility that opened in 1992. Pattie has developed health risk appraisals for use by at least two resellers of health risk appraisals. Additionally, she has developed lessons as a faculty affiliate of Duke University Medical Center for use with their patients and clients of the Center for Better Health on Duke’s campus. Later, Pattie enhanced the Vanderbilt Corporate Health Program by adding components to the basic program including the development of a health risk appraisal and point-of-care testing. Pattie also was able to develop systems, communication processes, and procedures for Vanderbilt to continue offering programs to local employers. Pattie has worked national accounts for United States Tobacco, and General Motors to conduct on-site programs. Other accounts include the Michigan Department of Public Health, McDonald’s, Seventh Day Adventist, Dollar General Corporation, Logan’s Roadhouse, Ingram Industries and Ingram Entertainment. Pattie earned a BS in Biology/Athletic Training at Southwest Missouri State University and MS degrees in Health Promotion and Marketing at Eastern Illinois University.
J. David Forbes, MD, ABIHM Chief Medical Officer
J. David Forbes, M.D., ABIHM, is the Chief Medical Officer of Corporate Health Partners. He is also the Founder and Director of Nashville Integrated Medicine. In addition to his practice in Nashville, he serves as President of AHMA, the American Holistic Medical Association. In 2008, Dr. Forbes was honored by the Nashville Business Journal as one of their “Health Care Heroes” for 2008. A native of Jackson, TN, Dr. Forbes received his M.D. from the University of Tennessee at Memphis in 1990. His internship and residency in Internal Medicine was completed at North Carolina Baptist Hospital/Bowman Gray School of Medicine (Wake Forest) in Winston-Salem, NC in 1993. He was board-certified in Internal Medicine in 1994, and also has over 10 years experience in Emergency Medicine. In the year 2000 he became a board-certified Founding Diplomate of the American Board of Integrated Holistic Medicine (ABIHM), one of only a handful of board-certified holistic practitioners currently in Tennessee. Required training for doctors board-certified in Holistic Medicine includes natural, herbal, nutritional, body/mind, emotional and spiritual approaches to healing and their integration with traditional medicine. The ABIHM certification is the only comprehensive, peer-reviewed integrative holistic medicine test standard for medical doctors. In addition, Dr. Forbes has over two decades of training and expertise in emotional stress reduction and relaxation techniques. He is a frequent conference/keynote speaker, a recognized expert in the field of holistic medicine, and a consultant to businesses in promoting transformative health change in the workplace.
Lisa L. Brandt, CPT, Program Manager|Wellness Coach
Lisa L. Brandt leads CHP’s development of progressive on and offsite wellness programming for CHP clients. Additionally she works to enhance one-on-one coaching methodology while managing programs and coaching individual participants and groups.
A rare 12+ year veteran in the worksite health and fitness industry, Lisa was previously the Wellness and Fitness Coordinator for Royal & Sun Alliance’s headquarters. There she led wellness solutions for 3,000+ employees at 36 branch offices countrywide and gained valuable insight into managing successful corporate wellness programs. Her tenure with R&SA allowed her to deepen her experience in every aspect of wellness from participant and facility management, including the creation of surveys to justify ROI findings. During her tenure, R&SA,‘s Health and Fitness Center was recognized by the North Carolina Governor’s Council on Physical Fitness and Health for exemplary wellness initiatives and overall commitment to their employees.
Brandt holds a degree from Central Michigan University and has furthered her education through advanced training. She has earned ACSM, ACE, Medical Exercise Specialist, certifications, among others. In addition to academic pursuits and experience, Brandt has competed in two Ms. Galaxy and one Ms. Nova competitions, taking challenges on the US Air Force obstacle course. Brandt also maintains a personal training business “No Limit” and volunteers for sport-specific training for youth groups.
Kathy Cash R.N., C.H.P.D Consultant for Work-site Health Promotion
A nationally recognized expert in the field of health promotion, Mrs. Cash has trained hundreds of health promotion managers and is author of How to Add Sparkle and Pizzazz to Your Health Promotion Program. In addition to many programs in corporate America, Mrs. Cash developed the first official wellness clinic for the United States Air Force and the first civilian prevention program provided to the Department of Defense. Mrs. Cash is a registered nurse, retired Air Force lieutenant colonel, and certified Health Promotion Director.
Theodore L. (Thad) Perry Ph.D. Consultant for Health Informatics
Dr. Perry has spent over 15 years gaining extensive experience within the health care arena, including behavioral health, experimental/research design, Medicare, Medicaid, commercial health care, and disease management. Prior to his work with Corporate Health Partners, Dr. Perry was V.P. of Health Informatics at American Healthways, the nation’s largest Disease Management organization. Earlier, Dr. Perry was Senior Statistician and Manager of Program Safeguards for the Medicare contract at CIGNA. Dr. Perry is considered one of the nation’s preeminent leaders in health care predictive modeling, publishing several articles and a book chapter as well as consulting with state and federal agencies. Dr. Perry received his Ph.D. and M.A. in Psychology from Vanderbilt University and his undergraduate degree from Clark University after attending the United States Military Academy at West Point for two years.
Robin F. Foust BS, PAHM Consultant for Employee Health Management
Ms. Foust is an award-winning specialist in health and human performance; disability and disease management; maternal and child health; and medical and quality management. Some of her strengths include population data analysis, strategic planning, process redesign, and healthcare management–including nursing and patient self-care management plans and content.
Ms. Foust serves on the faculty of the Bayer Institute for HealthCare Communication, where she trains clinicians to use communication techniques for enhanced outcomes in patience adherence, satisfaction, health status, and quality care. Among her awards is the Health Project’s C. Everett Koop Award in 1995 for her pioneering programs and outcomes achieved in maternity management and also in 1997 for outcomes achieved from her disease management programs. She also won the Lifetime Leadership Award from the Association for Worksite Health Promotion (AWHP).
