ROOTS Basic:  Building & Managing Programs

We build—and manage—an infrastructure and culture of health awareness to establish a great ‘ROOTS system’ for corporate wellness that’s participation-based and results-focused.  From moment one, Corporate Health Partners leads the process—interfacing with company leadership to set goals, create an account management strategy, establish effective communications and implement engaging programs.

Building & Managing Programs is the core of all ROOTS solutions and entails these nine key elements:

  • Culture Integration:  We assign CHP onsite account managers to lead program integration as well as all activities, events and promotions.  This approach helps infuse wellness into the corporate culture without adding to the workload of internal staff.
  • Communications & Promotions:  We create and manage communications efforts and promotions to keep participants informed and motivated.  All activities are promoted with our “three times—three ways” formula to get the word out.
  • Incentives:  We find ways to keep employees motivated so participation stays strong long term.
  • Team Coaching:  We establish and nurture wellness teams through onsite guidance.
  • Competitions/Fit Clubs:  We lead 3 to 4 organized, results-inspiring competitions annually (i.e. – Biggest Loser, Maintain Don’t Gain) backed by team coaching to inspire lasting change.
  • Education/Awareness Classes:  We offer a line-up of educational sessions with topics covering everything from Stress Management and Tobacco Cessation to Fitness 101 and Nutrition Basics.
  • Community Health Event Participation:  We lead organized employee involvement in community/citywide health events, such as heart walks, 5Ks, marathons or half-marathons.
  • Progress Reports:  We provide monthly reports to review accomplishments, clarify upcoming plans and state issues that need attention.
  • Continuous Improvement:  We continuously seek employee feedback in order to responsively make improvements that ensure ongoing participation and results.

ROOTS Plus:  Assessments for Baselines & Progress Measurement

You can’t manage what you don’t measure. That’s why Corporate Health Partners coordinates the scheduling of biometric screenings, nicotine/cotinine testing and Health Risk Assessments (HRA) to measure and establish baseline metrics for all aspects of wellness programs.  Continued measurement enables all parties to track progress and measure ROI—for the health of employees and the health of the company.
To ensure thorough, meaningful measurements, Corporate Health Partners provides

  • Biometric Screenings:  blood tests (lipids, blood sugar), blood pressure, body mass index, waist circumference, tobacco use
  • Health Risk Assessments (HRA):  survey to assess risky choices that drive chronic disease
  • Management Reports:  review aggregate data from the HRA and screenings and the associated potential for health and productivity improvement

ROOTS Max:  Personal Health Coaching

With the help of a health coach, wellness participants are up to 350% more likely to drop a bad habit for a good one.  That’s why CHP offers one-to-one coaching.  Our Wellness Managers are experienced, certified health coaches who are able to build credibility and trust among employees by being onsite regularly. Our onsite coaching is much more effective than the phone-in support of other providers.

  • Debriefing & Engagement:  One 30-minute private session between an individual employee and CHP wellness coach to discuss HRA & Screening Results, and set goals for improvement
  • Coaching Series:  Three 15 min private session each quarter with CHP wellness coach to create goals, establish accountability, follow-up, motivation, and problem solving
  • Management Reports:  review aggregate data from the coaching program
